DESCRIPTION OF INTERESTS

Professional Background
Dr. Joseph M. Mylotte is a graduate of the School of Medicine, University at Buffalo in 1973. He completed a residency in Internal Medicine and a fellowship in Infectious Diseases at the University at Buffalo. He joined the faculty of the School of Medicine, University at Buffalo in 1980 as an Assistant Professor of Medicine and in 1997 was promoted to the rank of Professor. Dr. Mylotte has served on the staff of all the major teaching hospitals of the University at Buffalo. Since 1993 he has been Chief, Infectious Diseases and Hospital Epidemiologist (until 2001) at Erie County Medical Center, Buffalo, NY. He also is an infection control consultant for 3 hospitals and 8 long-term care facilities in the western New York area. In addition to these activities, Dr. Mylotte has been an attending on the Acute Geriatrics Service at Erie County Medical Center since July 1997 and in June 2000 was appointed Clinical Director of the service. He is also an attending physician at the Rosa Coplan Living Center,
Weinberg Campus, Amherst, NY. Dr. Mylotte is a Fellow of the Infectious Diseases Society of America and a member of the Society of Healthcare Epidemiology of America, American Society for Microbiology, and Association of Practitioners of Infection Control.

Research Area
Infection in the elderly
Antibiotic use in geriatrics
Infection control in long-term care

Research Background
Since 1990 Dr. Mylotte’s research has focused on several areas: prediction models and outcomes related to infectious diseases and infection in nursing home patients. He has been particularly interested in the relationship between acute severity of illness and outcome of infections. He and his colleagues have studied the correlation between acute severity of illness and outcome among patients with S. aureus bacteremia (Infect Control Hosp Epidemiol 1996;17:165-168 and Clin Infect Dis 2000;31:1170-1174). He has collaborated in the development or validation of several prediction models. He and his colleagues have validated two models for predicting bloodstream infection (J Gen Intern Med 1995;10:82-88 and Infect Control Hosp Epidemiol 1995;16:203-209). He has also collaborated in the development of two different models for predicting pulmonary tuberculosis (Am J Resp Crit Care Med 1997;155:1711-1716 and Infect Control Hosp Epidemiol 1997;18:554-560). He has also developed a method for surveillance for
nosocomial C. difficile diarrhea that does not require chart review (Am J Infect Control 1998;26:16-23).

Dr. Mylotte has done extensive studies of the epidemiology of nosocomial infection and resistant organisms among patients undergoing acute rehabilitation for the first time. These studies had a particular focus on the spinal cord injured patient. These studies were done among patients admitted to the Acute Rehabilitation Unit at ECMC. The findings have been published recently (Clin Infect Dis 2000;30:425-432; Am J Infect Control 2000;28:291-297; and Infect Control Hosp Epidemiol 2001;22:83-87).

Research regarding infections and infection control in the nursing home setting has focused on several areas. He has developed a method for the utilization of a simplified statistical method for analyzing infection surveillance data in the nursing home setting (Infect Control Hosp Epidemiol 1996;17:101-107). He has also developed a method to monitor antibiotic use and cost in nursing homes (Am J Infection Control 1999;27:10-19). Since 1997 he has collaborated with Dr. Bruce Naughton in studies of nursing home-acquired pneumonia. Studies ongoing including evaluating the systemic inflammatory and innate immune responses of nursing home residents with aspiration pneumonitis and pneumonia; bloodstream infection in nursing home residents with special reference to methicilllin-resistant S. aureus; and developing indicators of antibiotic use in the long-term care setting.
